Patient Community Navigators work to reduce barriers to access to health and to educate communities about the resources available to them to improve their wellbeing and quality of life. They serve as a bridge between health care providers and the communities they wish to serve, in eliminating structures that hinder health equity in underserved communities.

They are also an important force in building and establishing trust in communities, particularly to address misinformation that widens health disparities in society.
